Buying online and collecting it in person
Click and collect fails at the handover rather than at the checkout.
The order is placed, the stock was showing as available, and the item is not on the shelf when somebody arrives — because it sold in person an hour earlier, or because nobody picked it, or because it is in a stockroom that a busy afternoon has made unreachable. Making it work means reserving the item at the point of order rather than hoping, and telling the customer it is ready rather than telling them it is ordered. Those are two different messages and stores routinely send the first as though it were the second. Beyond that, the ordinary details decide the experience: what identification is needed, how long you will hold something before releasing it, whether somebody else may collect on the customer's behalf, and what happens when they arrive outside the hours nobody mentioned. Each of those is a sentence on the confirmation and an argument at the counter if it is missing.

What is the most common failure?
Telling the customer their order is placed rather than that it is ready. Those are different messages and stores routinely send the first as though it meant the second, which is how somebody makes a journey to collect an item that nobody has picked off the shelf yet.
Does the stock need reserving?
Yes, at the point of order. Anything else means competing with the counter for the same item, and the customer who reserved it online is the one who loses, having already been told it was theirs. That is a considerably worse outcome than never offering collection at all.
What should the confirmation say?
What to bring, how long it will be held, whether somebody else may collect it, and the hours. Each of those is one sentence in an email and an argument at the counter if it is missing, and the argument is with a customer who did everything correctly.
